a39df7b2e7 Fix pack.toml format: move fields to top level 
The packwiz-installer expects name, author, version, and pack-format
at the TOML root, not nested under a [pack] section. This caused a
DecodingError on the client: "no value found for non-nullable parameter
'name'".

- index.toml: File index with SHA256 hashes
- mods/fabric-api.pw.toml: Fabric API 0.97.3+1.20.4 (Modrinth)
- mods/webstreamer.pw.toml: WebStreamer 1.5.0 (Modrinth)
- README.md: Setup instructions for server and client
